A busy GP network in Poole is seeking a friendly and organised Care Navigator to join their team. You will be the first point of contact for patients, helping to assess queries, manage online consultations, book appointments, and guide patients to the most appropriate healthcare services.

Key Responsibilities:

• Review patient queries and online consultation requests
• Escalate urgent concerns to clinicians
• Book appointments and update records accurately
• Support patients via telephone and messaging
• Signpost patients to suitable healthcare services

Skills & Experience:

• Excellent communication and organisational skills
• Calm and professional under pressure
• Strong customer service approach
• Healthcare or patient-facing experience desirable
